About the Case Management Directory™

Relative to the states it serves, the Case Management Directory™ stands as the most comprehensive home care directory ever developed. The Case Management Directory™ catalogs every medicare certified home health office serving the covered states. The CMD also lists DME, hospice, home oxygen, private duty, and personal care providers for hundreds of towns and cities. More importantly, the Case Management Directory™ exists specifically to fit the needs of discharge planners, case managers, care management workers, and other professionals in the business of finding homecare for their patients. Others are welcome to use the CMD as well.

The CMD will greatly reduce, if not eliminate, the need for case managers to call multiple home care providers asking if they serve a particular town. In the CMD, you will find a page for every town or city in the covered states. Under each town, you will find a list of home care providers that have claimed that town as part of their service area. Best of all, you can use the Case Management Directory™ free of charge.

Methodology

The Case Management Directory™ utilizes various public resources to identify every Medicare certified home health agency serving covered areas. Public resources were also used to identify hundreds of MDE, home oxygen, private duty, hospice, and personal care providers. CMD invited all such providers to submit forms showing their location and preferred contact information. Providers who did return the form and other providers selected based on telephone interviews can be found under the town where their office is located. Agencies requesting to be omitted from the Case Management Directory™ or agencies indicating they do not take referrals from hospitals are not shown.

The Case Management Directory™ also invited these providers to list every town they serve. The CMD shows those providers under additional towns for a fee paid by the provider. Not all providers chose to show their service area to case managers.

Why Not Counties?

To better serve case managers, the CMD does not list counties. The majority of home health providers have a county in their service area that they do not serve entirely. Therefore, listing counties puts case managers back in the inconvenient position of having to call more than one provider asking if the provider serves the town in question. By listing only towns, the CMD enables the case manager to call one provider and say, “I see that you serve the town of (blank).”

The CMD invited providers to submit lists of cities and towns they serve. When providers listed counties, that information was often not used. Specifications of regions (i.e., “Tri-City Area” or “Mountain Empire”) were also not used. If the provider listed a town not previously cataloged in the CMD, that town name was compared to the records of the United States Postal Service. If the Postal Service recognized the town in question, the town was added to the list of towns cataloged in the CMD. If the Postal Service did not recognize the town in question, that listing was not included in the CMD. Paid listings will not be refunded based on the submission of towns not recognized by the United States Postal Service.
